Consent Form & Post Care Instructions

Procedure Description

Skin tag/Mole/Skin Wart/Cherry Angiomas removal at La Bella Day Spa / La Bella Laser & Slimming may involve one or more of the following techniques:
● Electrocautery (burning)
● Non-surgical topical herbal cauterization (cream with active ingredient of cashew nut oil, herbal and fruit extracts)

These methods are used to safely remove benign skin tags, moles, and cherry angiomas. The procedure is considered cosmetic unless biopsy is indicated.

Risks & Side Effects

I understand that possible risks and side effects include, but are not limited to:
● Redness, swelling, or irritation at the treatment site
● Minor bleeding or scabbing
● Scarring or changes in skin pigmentation
● Temporary or permanent skin sensitivity
● Risk of infection (rare)
● Possible regrowth of the skin tag or need for additional treatments

I acknowledge that outcomes vary from person to person and that complete removal without any mark is not guaranteed.

Pre-Procedure Disclosure

I certify that I have disclosed all relevant medical information, including:
● History of keloids or poor wound healing
● Use of blood thinners or clotting disorders
● Allergies to antiseptics or topical anesthetics
● Diabetes or any condition affecting healing
● Pregnancy or breastfeeding status

Post-Procedure Instructions

I understand that proper aftercare is essential to reduce the risk of complications. I agree to:
● Keep the area clean and dry for 6 hours
● Avoid picking or touching the treated area
● Avoid sun exposure to the treated area until healed
● Apply Neosporin Ointment two times a day for 7 days
● Schedule a follow-up in 2 weeks if touch-up is needed (additional fees may apply)

Consent for Treatment

I authorize the licensed professionals at La Bella Day Spa / La Bella Laser & Slimming to perform skin tag / mole / Skin Wart / Cherry Angiomas removal using the method(s) deemed most appropriate.

I understand and agree that:
● This is an elective, cosmetic procedure
● I have had the opportunity to ask questions
● I have been informed of the treatment along with the expected benefits, risks, and possible consequences
● No guarantees have been made regarding results
● I may refuse or withdraw consent at any time
● Clinical photos may be taken for documentation, with confidentiality maintained
● I shall not hold La Bella Day Spa / La Bella Laser & Slimming, its owners, or staff liable for any unexpected outcome and waive all claims of any nature for said unexpected results
